In Home Care Services Can Include

In home personal care includes a wide range of services to ensure you and your loved one are safe and comfortable at home. Service can include showering, bathing and toileting assistance, meal preparation, laundry, shopping, light housekeeping, and more. Personal care services are provided by certified Personal Support Workers (PSWs) and supervised by Registered Nurses and Kinesiologists.

A Personal Care Plan

Our trained staff will meet with you to create a personalized care plan with your family and care team to ensure that your needs are being taken care of and to help ensure everyone is working together. This care plan includes a needs assessment, identification of goals and supports to help achieve these goals and emergency preparedness planning. This plan can be shared with members of the care team and a personal on line portal is available for all family members to receive updates, schedule and monitor visits, managing billing and more.

24 Hour Care, Overnight Care and Respite Care

Our services include 24 hour overnight staffing to ensure that you are safe at night and to respond in the event of an emergency. Respite services that allow caregivers a break are also available. In addition, we provide emergency response services. Remote health monitoring, tele-health services, virtual visits and Care Connect Services are also available.

Professional Nursing Care

At Stonebridge, all our services are overseen by a Registered Nurse and professional nursing care is available to help you manage age related health concerns. Our nursing services can help you with concerns regarding common health care challenges related to aging and will work collaboratively with your family and care team to help promote healthy aging and support independence in your home.

In Home Personal Care Services

Personal Care includes a variety of services dedicated to helping you maintain independence at home. Our compassionate and qualified PSWs will come to your home and help with daily activities such as:

Help around the home, including meal planning and preparation, grocery shopping, light housekeeping, laundry, safety inspection of the home and help to resolve concerns
Help monitoring and managing medications, including medication pick up and delivery, assistance with storage and organizing of medications and reminders to ensure proper usage of medication
Help with personal hygiene including assistance with bathing, personal grooming, toileting, and dressing
Personal in home exercise program including exercise activities, falls prevention risk assessment and prevention training, mobility assistance including lifts and transfers
In home foot health assessment, foot care, falls risk assessment and prevention help
Transportation to and from activities of daily living

Cost

Many of our services are covered under private insurance plans, provincial health care programs or on a fee for service basis. Our Care Coordinators will work with you to identify your personal and health care needs and to develop a plan that is best for you. We provide bundled care options and monthly memberships that allow clients to reduce costs. In some cases, subsidies may be available. Monthly billing and third party billing options are available.

Eligibility

Stonebridge Community Services provide services throughout the Tri-counties area (Oxford, Elgin and Norfolk) in Southwestern Ontario. Eligibility and availability of services vary across the area.
